Ozempic Injection Sites and Rotation Guide

Ozempic is a weekly subcutaneous injection, meaning it goes into the fatty tissue just below the skin, not into muscle. The three approved injection sites are the abdomen (at least 2 inches from the navel), the front of the thigh, and the upper outer arm. All three have similar absorption rates in clinical studies, so the choice largely comes down to personal preference and ease of access.

Rotation is not optional. Injecting into the same spot repeatedly causes lipohypertrophy, a buildup of fibrous scar tissue under the skin. Lumpy tissue absorbs the medication inconsistently, which can make your weekly dose less predictable. A simple rule: divide each site into quadrants and move one quadrant per week within the same site, then rotate to a different site.

Technique matters too. Pinch up a fold of skin, insert the needle at a 90-degree angle (or 45 degrees if you are very lean), inject slowly, then hold for 10 seconds before withdrawing. Slow injection and a brief hold reduce the chance of leakage and bruising. Never rub the site after injecting.

Temperature affects absorption. Ozempic should be stored in the refrigerator (2 to 8 degrees Celsius) and allowed to come to room temperature for about 30 minutes before injecting. Cold medication injected directly from the fridge is more likely to cause stinging and may absorb more slowly.

If you notice redness, swelling, or hard nodules at an injection site, avoid that area until it fully resolves, usually two to four weeks. Persistent reactions or pain should be reported to your prescriber. In rare cases, a site reaction can indicate an allergy to one of the inactive ingredients.
