Mounjaro vs Ozempic: What's the Difference?

Ozempic (semaglutide) and Mounjaro (tirzepatide) are both injectable medications used off-label for weight loss, but they work differently. Ozempic activates only GLP-1 receptors, which slow gastric emptying and reduce appetite. Mounjaro activates both GLP-1 and GIP receptors, a dual mechanism that appears to produce significantly greater weight loss in clinical trials.

In the SURMOUNT-1 trial, participants on the highest dose of tirzepatide lost an average of 22.5 percent of their body weight over 72 weeks. The STEP 1 trial for semaglutide showed roughly 14.9 percent weight loss. Both are substantial, but the gap is real and consistent across studies.

Side effects overlap heavily. Both medications cause nausea, constipation, and reduced appetite, especially during dose escalation. Some people report that Mounjaro nausea feels slightly more intense early on, while others find the opposite. Individual response varies widely and the only reliable way to know is to try, under medical supervision.

Nutritional strategy is nearly identical for both. Protein first at every meal, adequate fiber, limited ultra-processed foods, and staying hydrated.
